Heart Failure Re-Hospitalization Reduction Assignment

Description
Description
Phase 1 and 2 include the problem statement, goal, evidence of completion and plan: your project objectives. Students must post drafts for Phase 1 and 2 to their individual project thread discussion board. The purpose of the draft period is to provide time for feedback from faculty and peers allowing you to make corrections and improvements to project assignment before grading.
Project threads should be visited by peers and used to offer insight to each other’s projects, this feedback process will facilitate learned principles for all involved. See Professional Project Exemplars handout for examples of Phase 1 and Phase 2.
PHASE 1 – Draft due by Sunday of week 1; Final version due Sunday of Week 2
Write the problem statement, determine the project goal, and identify evidence of completion. Review the following guidelines:
Problem Statement: Addresses the who, what, where, when, why, how. Clearly identify what the problem is, who it involves, where this is occurring, what are the root causes, what literature supports the problem, what data exists, and how can you address the problem.
Project Goal: The project goal must be one sentence which identifies the following:
Condition– Conditions (if any) under which the performance occurs. What are the important conditions under which the performance is expected to
occur?
Who – Who is the target learner?
Performance – Which one action verb from Bloom’s revised taxonomy best defines your expected action for the learner?
Criterion – Evaluation of action selected for performance. What is acceptable performance? Describe how well the learner must perform in order to be
considered acceptable. For example, “As evidenced by Chart audit reveals >90% compliance with policy over next four weeks” What is the quality or
level of performance that will be considered acceptable? And what is the expected time frame?
Evidence of completion: Right now, all you need to do is identify your evidence of completion. This can be a PowerPoint, policy, webpage, program, etc. Note: if you are doing a presentation then you must include your teaching plan/outline. The actual evidence of completion will be submitted week 7.
